Marge,
I've come accross this problem a couple of times. I considred it a 245 indexing oddity and because of its infrequency didn't give it much thought.
Sounds like it happens to you very often. Did you call the helpdesk? If III develops a patch, we (and other sites) may need it, too.

Here is a strange one and I have been wracking my head trying to track it down.
When our catalogers are overlaying the acquisitions level bib records,
everything is searchable and fine in the webpac. However when they create an
original bib record or merge two bib records, we find that nothing is
retrievable in the webpac regardless of index searched (with the exception of
advanced keyword searching.)
We found that by deleting the 245, saving the bib record, reentering the 245,
and then saving the bib record again, the bib records are then retrievable in
the webpac regardless of index searched. If we do the same procedure with
other MARC fields--e.g., a 6xx subject or 1xx author field--the bib record is
only retrievable by the subject or author index, respectively.
We don't think it is a load table issue because when we create an original
record within Millennium the same problem exists. Also, we do not think it is
an indexing issue after checking our indexing rules.
